Verse 12 & 13
sarvadvaranai samyamya
mano hredi nirudhyacha |
murdhnyadhayatmanah pranam 
asthito yogadharanam ||12||
Omityekaksharam brahma
vyaharanma manusmaran |
yah prayati tyajandeham
sa yati paramam gatim ||13||
Artha:
"Having closed all the gates, having confined mind in the heart, having fixed his life-breath in the head, engaged in firm Yoga, uttering Brahman, the one-syllabled 'Om', thinking of Me, whoso departs, leaving the body, he reaches the Supreme Goal".
Transliteration:
sa-rva-dvA-rA-Ni  sam-ya-mya
ma-nO  hRe-di  ni-ru-dhya  ca |
mU-rdhnyA-dhA-yA-tma-naH  prA-Nam
A-sthi-tO  yO-ga-dhA-ra-NAm  ||12||
Om-i-tyaE-kA-xa-ram  bra-hma
vyA-ha-ra-nmA  ma-nu-sma-ran |
yaH  pra-yA-ti  tya-ja-ndaE-ham
sa  yA-ti   pa-ra-mAm  ga-tim ||13||
Analysis:
Having closed all the avenues of knowledge and having concentrated thought in the lotus of the heart, and with thought thus controlled, he ascends by the Nadi which passes upwards from the heart, and then fixing life-breath in the head, he who utters the syllable 'Om', the appellation of the Brahman, and meditates on Me is sure to reach the Supreme Goal at the time of leaving the body.
